* - Dr. Gates (at Flight Level's discretion) will travel to the client site either by commercial air, or by the company airplane. Our experience has been that the use of the company's aircraft represents a cost similar to that of commercial flight. To be arranged with client hospital prior to commitment.

** - Lodging of acceptable quality to be secured at client's expense (including day before start of shifts to potentially day after). Flight Level reserves to reject lodging of unacceptable quality.

*** - Includes day before/day after shifts begin/end. Must include travel either to general aviation airport used or closest hub commercial airport.

****- Cancellation fee arranged prior to commitment. This fee is nonrefundable.
